We recommend logging in 5 minutes early so we can start on time. You’ll receive a secure link and instructions via email or text before your appointment.

Any recent symptoms, concerns, or questions you want to discuss
Your recent blood pressure, blood sugar, or weight readings
Your pharmacy information (if you might need prescriptions)
